 How do I request leave from work?

If an employee is going to be out for more than 5 consecutive days, a Leave of Absent Request Form must be filled out.  Leave can be requested for the following reasons: Medical Leave (self or family), maternity, Military, Disability, etc.  Leave forms are to be signed by employees, supervisors, and submitted to HR for approval.  

 I just found out I'm pregnant what do I do?

After pregnancy is confirmed by the physician, and when the employee feels comfortable in sharing the news with others, the principal or direct supervisor should be informed of the pregnancy and the approximate date of delivery by a supporting note from the doctor.   Human Resources should also be sent a copy.  At this point, HR will determine if you are eligible for FMLA.  Once determined, you will be sent FMLA paperwork (Eligible or Denied), and a Leave of Absence Request Form is also needed for HR records.  If you have questions regarding your benefits, please contact the Finance Department.  Maternity Leave Information
