1095 Tax Information
TAX INFORMATION: IMPORTANT HEALTH COVERAGE TAX DOCUMENT (1095-C FORM)
According to new Federal requirements, you may receive a copy of your Form 1095-C upon request. This means that Alexander County Schools is no longer required to automatically send you a 1095-C.
If you would like a copy of your Form 1095-C, please access our benefits portal at https://nc.secure-enroll.com/go/NC-SHP and download your Form 1095-C electronically.
Alternatively, if you need us to mail your Form 1095-C, please contact us as noted below. We will send your form no later than 30 days after we receive your request.
If you are no longer employed by Alexander County Schools or you retired in 2025, you will
receive your Form 1095-C in the mail.

Where do I find my electronic 1095-C form?
- Log into your SHP member benefits portal.
- At the top of the page, hover over "benefits" tab and click "1095 tax documents" on the righthand side of the banner under "benefit reports"
- Here, you will see list of all 1095-C documents for each year of your tenure with this group.
- From this page, select "download" of the document you want a copy of.
- A box with "Please agree to the Terms & Conditions" will appear, so select "I agree"
