What documentation is needed to register a New 1st - 12th grade student?

• You will have the option to upload these documents or provide a copy at the school
  • Parent or Guardian Driver’s License
  • Student Birth certificate or other proof of age (ex. Student ID, Baptism Certificate, Passport, etc.)
  • Immunization record (originals from the doctor or health department)
  • Proof of residency (ex: current utility bill, mortgage payment, vehicle registration)
    • Board Policy Code: 4120 DOMICILE OR RESIDENCE REQUIREMENTS
  • NC Health Assessment completed by parent/guardian and healthcare provider (provided to the school within the first 30 days of enrollment)
• You will need to provide a copy of these documents at the school
  • Last report card / withdrawal documentation or transcript from previous school (if available)
  • Sports physical if the student plans to play a school sponsored sport (grades 6-12 only)
